In April 2026, information about the accessible information standard was added to Section 2, Providing Information and Advice. In addition, links to the Team Suitability Decision Support Tool, Urgent or Interim Support Decision Tool, and Allocation Support Tool were removed throughout, as these resources are no longer available.

In April 2026, the link to the Care and Support Plan Sign Off Tool was removed from Section 3, Signing Off a Plan (Guidance for Authorisers) as this resource is no longer available.
