There are three teams within the front door who work together to manage concerns jointly. Open, honest, collaborative communication between practitioners and clinicians within these teams is instrumental in decision making to achieve the desired outcome for specific individuals.

The Initial Contact Team are the first point of contact for all Adult Social Care telephone calls and emails.
They are responsible for:
- Conducting strengths-based conversations;
- Providing information, advice and guidance;
- Building knowledge of, and connections with local community resources;
- Providing initial screening of contacts to determine if there is likely need for further assessment.
The Community Intervention Hub (CIH) is designed to enhance the way residents access support. The transformation introduces new choices and pathways for individuals contacting the Front Door, ensuring they receive the right level of intervention at the right time. The CIH takes a preventative, strengths-based approach, aiming to offer timely and proportionate intervention. Through a multi-disciplinary model, the CIH integrates health, social care, and community-based services to promote independence and well-being.
The Safeguarding Hub within the Adult Social Care Front Door is responsible for responding to safeguarding concerns, ensuring a swift and effective response to safeguard adults at risk. The hub operates as a central point for receiving, screening, and managing safeguarding referrals. Working collaboratively with health, police, and other partner agencies to promote safety and well-being.
