Team 4: Orpington and Crays

There are four community locality teams, for the most part the teams will work with people whose GP surgeries are located within the team's geographical boundaries. There are some exceptions to this which have been made to balance workload for the new teams. These teams provide a range of services to adults within that area, carrying out the full range of functions under the Care Act 2014 and the Mental Capacity Act 2005. This includes assessment to establish needs, care and support planning, undertaking reviews, providing a duty function, safeguarding, reablement assessment, complex care, mental capacity assessment and best interest decision making.
