Coordination and Review Team

The Coordination and Review Team are responsible for completing planned annual reviews, and reassessments, for all individuals under Adult Services, excluding those whose needs are met by the Mental Health Service or the Learning Disability Team. The Coordination and Review Team also carry out safeguarding casework and manage Extra Care Housing tenancies, both long term tenancies and step-down flats to facilitate hospital discharge.
