Hospital Social Work

The Hospital Discharge Social Work Team support adult residents of Bromley with initial assessment following discharge from hospital. This includes:

• Supporting Bromley's brokerage and placements team by providing information on cases;
• Working on social admissions to the Emergency Department;
• Being involved in Multi-Disciplinary Team case work;
• Linking to other contributors to preventative work in relation to Occupational Therapy led services, Integrated Care Network (ICN) social work staff and Rehab care management workers.
